Athiran is a 2019 Indian Malayalam -language psychological horror film directed by debutant Vivek that got released on 12 April 2019. Fahadh Faasil and Sai Pallavi in the leading role. The film was produced by Ayoob Khan.

Design
The movie starts at 1967 when Lakshmi (Shanthi Krishna) sees Nitya (Sai Pallavi) tangling with a thread while seeing that the rest of the household have been killed. The story shifts 5 years later to a high range location where Dr. M. K. Nair (Fahadh Faasil), a psychiatrist from Trivandrum Medical College, is travelling in Jeep to check about the activities in the mental hospital run in the high range bungalow.The story revolves around how Dr. Nair looks after the patients where he meets Dr. Benjamin (Atul Kulkarni) who is the owner of the hospital. It later comes to the notice that although 5 patients were admitted there was a sixth patient called Nitya unknown in the records. As Dr. Nair demands to see her, he is shown the person.
The plot revolves around her and her back-story. It is later revealed that Dr. Nair is actually Vinayan, Nitya's cousin who was impersonating his own psychiatrist who was treating him for schizophrenia and had delusions, hallucinations and extreme paranoia. It is Vinayan and not Nitya in fact, who murders their family after their uncles murder Nitya's father. The film ends with the real Dr. Nair swearing that he is always right behind his patient, Vinayan no matter what move he makes.
